Background: FK 506 antigen is a novel macrolide with similar but more potent immunosuppressant activity than cyclosporine. FK 506 is used for primary or rescue immunosuppression in patients after solid organ transplantation. Clinically relevant FK506 side effects are nephrotoxicity, neurotoxicity, diabetes mellitus and hypertension. Most of these side effects are more closely related to FK 506 blood levels than dosage; therefore therapeutic drug monitoring (TDM) of FK506 levels is a prerequisite for therapy. Whole blood is the matrix of choice for FK 506 TDM. The whole blood target level range aimed for in clinical practice is between 3.0 and 20.0 ng/ml and depends on the patients liver function, the type of organ transplanted and the time after transplantation.
